Patient Paperwork

Our new patient paperwork can be downloaded so that you may fill it out at home and bring it with you in order to save time at your first visit. If you have any questions please give the office a call. Thank you.

Doctor Consultation

The Doctor will consult with you to further evaluate your health and to determine which type of evaluation and examination procedures will benefit your specific needs.

Examination

The Doctor will examine your spine and nervous system through state-of-the- art examination procedures including:

» X-Ray
» Computerized surface EMG

Report of Findings

This is where you will be educated to the fullest extent on your condition and exactly how we can help you. Your consultation, examination, EMG and X-Ray findings will be thoroughly reviewed and treatment recommendation's will be made based on these findings. All finances will be discussed before starting care. We will work with you any way we can to make it affordable so that you get the care that you need.
